The England full-back watched the visit of relegation-threatened Parma from the San Siro terraces.
Milan were below par for much of the game and entered stoppage time trailing 2-1.
After having a Strahinja Pavlovic goal ruled out for offside, Reijnders and then Chukwueze sparked pandemonium in the Italian capital.
But despite the initial euphoria, tempers flared on the field following the final whistle.
Davide Calabria was confronted by his own manager Sergio Conceicao, who had run onto the field.
The Portuguese boss remonstrated with the AC captain, with the pair exchanging shoves.
Milan players quickly got in between them, but Conceicao continued to gesture and shout towards his skipper.

One stunned fan wrote on X: “Calabria has been a Milan player since birth. Conceicao is leaving after the season. I stand with my captain.”
Another said: “Everyone going against Calabria.. I’m not his biggest fan but this isn’t a good look. Coach shouldn’t behave like that.”
While a third added: “I stand with my captain. He told [Conceica] to use better tactics I bet.”
But a fourth countered: “Drop him to Milan [B team].”
Calabria, who plays at Walker’s right-back position, has been with Milan since the age of 9.
He was made captain two years ago and has appeared over 270 times for the club.
Conceicao subbed Calabria off during Sunday’s comeback, replacing him with striker Luka Jovic as Milan chased the game with 13 minutes to go.
